Purpose | To provide a single point of entry for older adults, adults with disabilities, their families, and friends to obtain needed information and assistance |
Service | Through this program citizens receive needed information on services available and assistance in obtaining benefit programs. Referrals are made to supporting services and agencies. Care access/case management of each situation is done by an in-depth assessment of the individual's needs. Follow-up to ensure delivery of service is part of the program. Education regarding the availability of community resources, services and items of importance to the older adult, and adults with disabilities population is an integral part of the program. |
