Purpose	We are a ministry of Lutheran Social Services, providing health care and related services to those of all ages throughout Carroll County who are striving to achieve their highest possible quality of life, health, and comfort at home.
Service	We are a not-for-profit, Medicare-certified home health agency. Services available in the client's place of residence include: skilled nursing care to assist with treatments, patient education, lab collection, wound care, maternal/infant care, and IV therapy services. Certified home health aide services are available to assist with personal care under the supervision of a skilled clinician. Under comprehensive rehabilitation services, a team provides physical, occupational and/or speech therapy in the client's home. Medical social workers assist the client and family accessing community resources. Telehealth monitoring is part of our standard of care and provides daily vital signs readings to a clinician, which gives the client and family ongoing reassurance. The staff is also available to participate in community health screening or to act as speakers for local events. Fees	Payment: Medicare, Medicaid, private insurance
Ages Served	Serves all ages from infants to geriatric patients.
Area Served	Serves Carroll, Frederick, and Washington counties in Maryland; and Adams, York, southern Cumberland, and Franklin counties in Pennsylvania.
