Purpose | To enable older adults and people with disabilities to live in their own homes even though their advanced age or physical disability would warrant placement in a nursing home. |
Service | Depending on the needs of the individual, the following services might be applicable: personal assistance services, personal emergency response systems, technology, environmental assessments, accessibility adaptations, consumer training, nurse monitoring,supports planning and home delivered meals. |
Eligibility | Applicants must meet financial eligibility requirements and receive Community Medicaid. In addition, applicants must meet Medicaid's long-term care admission criteria (nursing home level of care). |
